The Queen Angelfish has a vibrant blue or greenish-blue body with scales outlined in yellow, and it has yellow fins. A striking deep blue spot, resembling a crown, is present on its forehead, speckled and ringed with a lighter blue. Juveniles have a darker blue body with curved blue or pale vertical bars. A dark blue band runs through the eye, outlined by thin blue lines, and their tail and snout are yellow.

This beautiful Angelfish is sometimes seen in pairs and is not very shy. It eats mostly sponges but also eats algae and other small marine animals. Juveniles sometimes act as cleaners, removing parasites from other animals. During spawning, a male courts several females, and pairs swim upward together to release eggs and sperm into the water.
