The Blackear Wrasse has a small, dark spot behind the eye and another dark mark near the back of the dorsal fin. The body is relatively long and thin. Adults in their terminal phase have green bodies with diagonal pink or red bands forming a V on the tail. The initial phase is green to yellow-green, while juveniles are pale with dark bars on their back.

This Wrasse is shy and often found alone swimming over the shallow reef or seagrass beds. During the day, it hunts for small invertebrates on the sandy seafloor, such as shrimp, crabs, and sea urchins, by turning over rocks and coral rubble.
