The Blackear Wrasse has a small, reddish spot behind its eye and another dark mark near the back of its dorsal fin. Its body is fairly long and thin. Terminal-phase adults have green bodies with diagonal pink or red bands that form a V on the tail. Initial-phase adults are green to yellow-green. Juveniles are pale with dark bars on their back.

The Blackear Wrasse is shy and often found alone swimming over the shallow reef or seagrass beds. During the day, it hunts for small invertebrates on the sandy seafloor, such as shrimp, crabs, and sea urchins, by turning over rocks and coral rubble.
